Please ensure the following is done before turning in a sports physical:

1. Doctor has signed AND dated the physical
2. Doctor has checked cleared or not cleared
3. Parent and student have both signed and dated the pre-medical history form.

All copies must be submitted digitally and none will be accepted in person.
